Orientation Forms

Please complete all Orientation paperwork by logging into UCI Law Student Forms. All forms must be submitted by 5:00pm on Monday, August 14, 2023.

Below is a list of forms you will see in the Student Forms system:

  1. Career Interest Survey: Responses to this survey will be used to match you with a career counselor in the Career Development Office for your 1L year.
  2. Recording Policy Form: Please sign to acknowledge that you are aware of the Law School's Recording Policy. Links to recorded classes will be released only to students who have a signed acknowledgment on file.
  3. NCBE Agreement Form: UCI Law licenses certain bar preparation material from the National Conference of Bar Examiners (NCBE), for use in law school courses and during the bar prep study period. Please sign to abide by and acknowledge the NCBE’s licensing agreement
  4. Photo/Video Consent: There are many times we have great photo and video footage that we might want to use on the website or in other forms of social media, or to highlight your accomplishments.  Please let us know if we are allowed to use your image.
  5. Personal Pronouns: On this form, you may indicate your personal pronouns for informal use at UCI Law, including the option to include your pronouns on the online UCI Law student directory and on our class rosters. This is completely optional, so please do not provide any information you are uncomfortable sharing. 
  6. Consent to Release Student Information: On this form, you can designate individuals to whom we are allowed to release information in an emergency.
